Tournament Rules: Entry: $350 per Team per Tournament - Non–Refundable after Deadline date. $50.00 Charge for Returned Checks Mandatory Check-In with Site Director 1 hour before scheduled game time. Proof of Age, Proof of Insurance, and ASA or VTD Rosters (not the VTD My Team page) must be turned in to Site Director. Completion of this entry form signifies your agreement to abide by the decisions of the Tournament Director. All efforts will be made to provide a fair schedule to every team but demand for fields makes this difficult. No special requests for times or locations will be accepted. Tournament Director reserves the right to adjust game times and field locations and length of playing time if necessary.

NOTE:Teams need to be entered into the Tournament via VTD by the Sign Up/Required By date on VTD. Completed entry form must be sent into Phx ASA by the SignUP deadline date either email phxasajow@cox.net or fax 602-493-0401 and payment received no later than same week Friday by 5 PM. If NO Payment by Friday date your team will be dropped from the tournament.
